What to bring

Always bring#

  • Identification, and your insurance or membership details if you have them.
  • A list of every medicine you take, including ones bought without a prescription and anything herbal.
  • The names of any allergies and what happens when you are exposed.
  • Contact details for anybody else involved in your care.

Bring if you have them#

  • Letters, results, images or records from another practice.
  • A referral, if you were sent to us by somebody else.
  • Anything you have been using on the area concerned.

Fill this in: anything specific to your practice — an appliance, a device, a previous set of images, a pet's vaccination record, a stool or urine sample with collection instructions. Be exact about how a sample should be collected and how fresh it has to be.

Forms#

Fill this in: which forms a new patient completes, whether they can be filled in beforehand on https://webrzn.ru/, and how long they take. A form done in advance is ten minutes of the appointment returned to the actual reason for coming.

If you are bringing somebody who is not yourself#

A child

Fill this in: who may give consent, what identification is needed, and whether both parents need to agree to anything.

An adult who needs support

Fill this in: what you need from somebody attending on another adult's behalf, and what can and cannot be discussed without the patient present.

An animal

Fill this in: carrier, lead, muzzle, vaccination record, and whether to withhold food before the appointment. The food question is the one most often missed and most often matters.

Bring the medication list even if you think it is irrelevant. Interactions are the most common avoidable problem in any practice, and the medicine nobody mentioned is almost always the one that mattered.

What not to bring#

Fill this in: anything you would rather patients left at home — valuables, other pets, large bags. Say why in half a sentence and it reads as practical rather than as a rule.
